We are contracted with many Managed Care insurance companies, including Medicaid. Keep in mind that insurance plans vary, and each has its own requirements and restrictions. Please check with your insurance company to confirm provider status and services covered. To simplify billing and keep medical cost down, co-pays, and payments for deductibles and non-covered services are due at the time of your appointment. Balances remaining after insurance payment or non-payment will be due 30 days from the date of service. We have provided a list of many of the insurance plans our physicians can see patients as in-network providers. If your insurance company is not listed, please contact our office or your particular company to ascertain provider status.

As of January 2000, legislation was passed that requires all HMO's to provide their subscribers with an out of network option for their physician. Contact your insurance company for more information on this option.
